hearing test assessment, you provide your Beltone hearing care professional with important information needed to make the right recommendations for your lifestyle.
To open the PHHA form, click here or on the image to the right. Once opened, select your answers to the questions. When finished, simply print out the completed form and take it to your local Beltone Hearing Test Center.
Your Beltone hearing care professional will review the results of your PHHA assessment with you and will give you a hearing test screening to find out whether or not you have a hearing loss. He or she will then consult with you on next steps for your hearing care needs and which type of hearing aids you will need.
